Summit Adult Services

Below is a list of Summit's Adult Division services. Please call 716.629.3400 to learn more about any of these services.

Day Habilitation

S.T.E.P.S. (Success Through Engagement and Positive Supports)  

With three locations across Western New York, S.T.E.P.S. is a day habilitation program where you will have the opportunity to make a new group of friends and participate in lots of community outings. S.T.E.P.S. participants volunteer for different community organizations in the area, engage in leisure activities, and participate in various skill-building opportunities that might help you get a job.

Eligibility:

  • Participant must have exited high school
  • Must be eligible under the New York State Office for People with Developmental Disabilities (OPWDD)
  • Service includes transportation to and from program, and to community activities throughout the program day.
  • This service is provided in a community setting, and individuals must be able to safely participate in this type of setting
  • This service is funded by the New York State Office for People with Developmental Disabilities (OPWDD) and provided at no direct cost to individuals who are eligible

Pre-Vocational

ACCES-VR

The Summit Center is authorized to provide a number of employment services for individuals that receive support from the New York State Office of Adult Career and Continuing Education Services – Vocational Rehabilitation (ACCES-VR).

Eligibility:

  • Individuals who are high school aged to adult may be eligible
  • Confirmed diagnosis of a disability
  • Deemed eligible by ACCES-VR

P.A.C.E. (Pre-Vocational and Community Experience)

Through out collaboration with the University at Buffalo’s Campus Dining and Shops, P.A.C.E. offers our interns real work experience. This pre-vocational training program will help get you the necessary work experience and job skills to obtain a job.

Eligibility:

  • Participants must be 18 years of age or older.
  • Must be eligible under the New York State Office for People with Developmental Disabilities (OPWDD)
  • This service is funded by the New York State Office for People with Developmental Disabilities (OPWDD) and provided at no direct cost to individuals who are eligible
  • Basic communication skills
  • Ability to be left unsupervised for brief periods of time
  • Independent toileting and feeding skills
  • Maintains own transportation to and from training location

OPWDD – Supported Employment

If you are eligible under OPWDD, we will help you find and maintain gainful employment. We will work with you directly to make sure where you work is the right fit for you.

Eligibility:

  • Participants must be 18 years of age or older
  • Must be eligible under the New York State Office for People with Developmental Disabilities (OPWDD)
  • This service is funded by the New York State Office for People with Developmental Disabilities (OPWDD) and provided at no direct cost to individuals who are eligible

Care Coordination Services

Person-Centered Services (https://personcenteredservices.com/)

Person Centered Services provides Care Coordination for over 17,000 people with intellectual and developmental disabilities in 18 counties throughout the Western New York, Finger Lakes, and Southern Tier regions. Their goal is to help individuals and families navigate a very complex system.

OPWDD Front Door Information Sessions

The Front Door is the way the Office for People With Developmental Disabilities (OPWDD) connects people to the services they need and want.

Once you “enter,” a person-centered planning process begins which helps you learn about and access service options taking your needs and desires into consideration.

It will also give you the chance to direct your own service plan or help your family member or loved one as they direct theirs. This information session will outline the process of to become eligible for OPWDD supports and services, the types of supports and services available and where you can go to get assistance.

*If you haven’t already, you MUST attend a Front Door session and receive a certificate for your child to be eligible for services after they graduate from school.

Guardianships, Wills, and Special Needs Trusts Webinar

On Wednesday December 9, 2020, The Summit Center will host an online presentation by Bruce A. Goldstein, outlining the topics of guardianship, wills, and special needs trusts.

General Information:

According to New York State law, when a child reaches age 18, that individual is considered legally competent to make personal, medical, and financial decisions. An intellectual or developmental disability may make it difficult for an individual to make those decisions and manage personal affairs, introducing potential risks to that individual’s welfare.  Mr. Goldstein’s presentation will cover options available to parents and caregivers to protect the person, property, or both, of an individual with an intellectual or developmental disability. This webinar will also cover the topic of qualifying for, and maintaining, government benefits (particularly SSI and Medicaid).

You should attend if:

Your child has a disability and…

-is 15-21 years old, but you have not yet appointed a guardian.

-is under age 15 and you want to prepare for the guardianship process.

-you have guardianship, but you are interested in a will or trust.

About the presenter:

Bruce Goldstein, Special Counsel at Kenney Shelton Liptak Nowak LLP, has more than 40 years of experience practicing law. He concentrates his practice in the areas of Special Education, Special Needs, Estate Planning, Not-for-Profit Corporations, and Health Law. He heads the department at KSLN that serves individuals, their families, and community organizations and employees that serve people with special needs in the Education and Human Services fields.
